Priority (0–325)
The priority score is a consolidated metric that condenses several individual signals into a single, action-guiding ranking: technical severity (CVSS), exploitation likelihood (EPSS), known-exploited status (KEV) and the context of the affected asset. The aim is to answer the only truly important question in vulnerability management: what first?

Exploit maturity
An exploit is code or a technique that specifically uses a concrete vulnerability to force unintended behaviour — such as executing code or escalating privileges. It is the tool that turns a theoretical flaw into a practical attack.

XIEM Effective CVSS
XIEM® is the tiered managed-security operating model from NEOSEC and a registered trademark of NEOSEC GmbH. It extends classic SIEM — the central collection, normalisation and correlation of security-relevant events — with endpoint forensics, network visibility and deception technology, and consolidates them into three building tiers: Sentry, Orchestrate and Command.
Read in glossary- 9.8 Methodology
CVSS v3
CVSS is the globally established, open standard for assessing the technical severity of a vulnerability on a scale from 0 to 10. It describes how easily a flaw can be exploited and what damage it can cause — but not how likely its actual exploitation is. CVSS and the complementary EPSS deliberately answer different questions.

CVSS v4
CVSS is the globally established, open standard for assessing the technical severity of a vulnerability on a scale from 0 to 10. It describes how easily a flaw can be exploited and what damage it can cause — but not how likely its actual exploitation is. CVSS and the complementary EPSS deliberately answer different questions.

EPSS score
The EPSS score is the absolute model output for an individual vulnerability: the estimated probability of real-world exploitation within the next 30 days, expressed as a value between 0 and 1 (i.e. 0 % to 100 %). A score of 0.42 means „the model estimates a 42 % chance of real-world exploitation within 30 days“.
Read in glossary- 82.2 %
- High — model estimates ≥ 50% chance of real-world exploitation within 30 days.
EPSS percentile
The EPSS percentile is the relative position of a vulnerability within all EPSS-scored CVEs, expressed as a value between 0 and 1. A percentile of 0.90 means „this CVE has a higher EPSS score than 90 % of all other scored CVEs“. Unlike the EPSS score, it is not a probability — but a positioning within a ranking.
Read in glossary- 99.65
- Critical — this CVE ranks in the top fifth of all CVEs scored today (rank ≥ 80%).
KEV status
The KEV catalogue is a list maintained by the US authority CISA of vulnerabilities for which active exploitation in the wild has been confirmed. It answers not the question „how severe?“ but „is it already being attacked?“ — thereby sharply prioritising patching.

Response and mitigation
Why act now?
Prioritisation rationale
NetScaler ADC and Gateway sit on the perimeter as the remote-access and load-balancing tier. Network-reachable command execution without authentication or user interaction therefore means full takeover of a box that holds credentials, live sessions and a path into the internal network. EPSS currently models only a moderate exploitation likelihood, but KEV inclusion confirms real-world attacks, and NetScaler flaws have historically been mass-scanned quickly and used for webshell persistence. Treat internet-facing instances as top priority: patch within 24 h and run a compromise assessment. CISA does not flag ransomware use, but that should not lower urgency for exposed systems. Purely internal instances with no reachable management or Gateway interface can follow in the regular emergency patch window.
Runbook step 1
Immediate response (0-24 h)
- Inventory: Enumerate every NetScaler ADC and NetScaler Gateway instance (MPX, VPX, SDX guests, cloud marketplace) and record firmware via
show ns version. Affected are builds before 14.1-73.37 and 13.1-64.23, plus FIPS/NDcPP builds before 13.1-37.279 (listed in the context as "13.1.37.279") and 14.1-73.37 FIPS. - Patch: Upgrade internet-facing Gateway and ADC appliances first to 14.1-73.37 or 13.1-64.23. Move FIPS/NDcPP systems to 13.1-37.279 or 14.1-73.37 FIPS. For HA pairs, upgrade the secondary first, then fail over.
- Preserve evidence: Patching does not remove implanted persistence. Before upgrading, image or copy
/var/log/,/var/nslog/and the web roots so you can run a compromise assessment. - Terminate sessions: After upgrading, run
kill aaa session -all,kill icaconnection -all,kill rdp connection -all,kill pcoipConnection -allandclear lb persistentSessions. - Rotate credentials: If compromise is suspected, rotate
nsroot, local admin accounts, LDAP/RADIUS bind accounts, service credentials stored inns.conf, and TLS private keys. - Reporting: Check NIS2 reporting duties if compromise is confirmed. An early warning to the national CSIRT/authority is due within 24 h.
- Inventory: Enumerate every NetScaler ADC and NetScaler Gateway instance (MPX, VPX, SDX guests, cloud marketplace) and record firmware via
Runbook step 2
Mitigation layers
- Network segmentation: Restrict management interfaces (NSIP, and any SNIP with management access enabled) to a dedicated admin VLAN or jump host. Block TCP 22/80/443/3010/3008 from everywhere else on the upstream firewall.
- Attack surface: The context does not specify the entry vector, so treat every externally reachable vServer (Gateway, AAA, LB with GUI) as exposed. Disable unused vServers and temporarily restrict Gateway access by geo or IP allowlist.
- Egress filtering: Allow outbound traffic from the appliance only to known destinations (DNS, NTP, syslog, auth backends). Default-deny egress breaks payload staging and reverse shells.
- IPS/WAF: Enable vendor IPS signatures for CVE-2026-88771 (Suricata ET, Palo Alto, Fortinet) as soon as they ship. Until then, log and review requests to
/vpn/,/oauth/,/cgi/and/nf/auth/that carry oversized headers or parameters, or shell metacharacters (;,|,$(, backticks). - IAM/least privilege: Require centralised admin authentication with MFA. Keep
nsrootas a break-glass account only, with a strong vaulted password. - Lifecycle: Identify any release lines that are out of vendor support and plan migration, as no fix should be expected for them.
Runbook step 3
Detection rules
- Process ancestry (shell logs /
ps): Treatsh, bash, curl, python, perl, ncoropensslspawned byhttpdornsppeas usernobodyas high-fidelity. Sigma shape:ParentImage|endswith: '/httpd' AND Image|endswith: ['/sh','/bash','/curl','/python']. - File integrity: Hunt for new or modified files in
/netscaler/ns_gui/,/var/netscaler/logon/,/var/vpn/and/var/python/, e.g.find /netscaler/ns_gui /var/netscaler/logon /var/vpn -type f \( -name "*.php" -o -name "*.sh" -o -name "*.xhtml" \) -mtime -30. Also reviewcrontab,/etc/rc.netscalerand/nsconfig/rc.netscalerfor unexpected entries. - Web logs: Search
/var/log/httpaccess.logandns.logfor requests containing shell metacharacters or Base64 blobs, and for hits on newly dropped.phpfiles. SPL:index=netscaler sourcetype=citrix:httpaccess uri="*.php" status=200 | stats count by src_ip uri. - Network telemetry (Zeek/Suricata): Flag outbound connections from NSIP/SNIP to unknown external IPs, especially on unusual ports or with long durations (reverse shell). In Zeek
conn.log:id.orig_h in netscaler_ips and not id.resp_h in allowlist. - Crash indicators: Repeated
nsppecore dumps in/var/core/or/var/crash/may indicate failed exploitation attempts.

Classification under NIS2 and CRA
Rule-based classification from exploitation status and severity. It does not replace a case-by-case assessment.
NIS2
NIS2 is EU Directive (EU) 2022/2555 to raise the level of cybersecurity across the Union. It substantially broadens obligations compared with the first NIS directive: more sectors, mandatory risk management, staged reporting duties and personal accountability of senior management.
Read in glossary- Actively exploited: treat the vulnerability as a priority in vulnerability handling under Art. 21(2)(e) NIS2. If it is exploited in your environment, check the reporting obligation under Art. 23 (early warning within 24 hours).
CRA
The CRA is EU Regulation (EU) 2024/2847 setting horizontal cybersecurity requirements for „products with digital elements“ — from industrial controllers and software to connected consumer devices. For the first time manufacturers must ensure security across the entire product lifecycle, handle vulnerabilities and provide security updates.
Read in glossary- Since 11.09.2026, manufacturers of affected products with digital elements must report actively exploited vulnerabilities under Art. 14 CRA, with an early warning within 24 hours via ENISA's single reporting platform.
ISO/IEC 27001
ISO/IEC 27001 is the internationally leading standard for an information security management system (ISMS). It describes a risk-based, process-oriented framework to systematically plan, implement, review and improve information security.
