Priority (0–325)
The priority score is a consolidated metric that condenses several individual signals into a single, action-guiding ranking: technical severity (CVSS), exploitation likelihood (EPSS), known-exploited status (KEV) and the context of the affected asset. The aim is to answer the only truly important question in vulnerability management: what first?
Read in glossary- 243.44 Methodology
Exploit maturity
An exploit is code or a technique that specifically uses a concrete vulnerability to force unintended behaviour — such as executing code or escalating privileges. It is the tool that turns a theoretical flaw into a practical attack.

XIEM Effective CVSS
XIEM® is the tiered managed-security operating model from NEOSEC and a registered trademark of NEOSEC GmbH. It extends classic SIEM — the central collection, normalisation and correlation of security-relevant events — with endpoint forensics, network visibility and deception technology, and consolidates them into three building tiers: Sentry, Orchestrate and Command.
Read in glossary- 7.8 Methodology
CVSS v3
CVSS is the globally established, open standard for assessing the technical severity of a vulnerability on a scale from 0 to 10. It describes how easily a flaw can be exploited and what damage it can cause — but not how likely its actual exploitation is. CVSS and the complementary EPSS deliberately answer different questions.
Read in glossary- 7.8
EPSS score
The EPSS score is the absolute model output for an individual vulnerability: the estimated probability of real-world exploitation within the next 30 days, expressed as a value between 0 and 1 (i.e. 0 % to 100 %). A score of 0.42 means „the model estimates a 42 % chance of real-world exploitation within 30 days“.
Read in glossary- 81.9 %
- High — model estimates ≥ 50% chance of real-world exploitation within 30 days.
EPSS percentile
The EPSS percentile is the relative position of a vulnerability within all EPSS-scored CVEs, expressed as a value between 0 and 1. A percentile of 0.90 means „this CVE has a higher EPSS score than 90 % of all other scored CVEs“. Unlike the EPSS score, it is not a probability — but a positioning within a ranking.
Read in glossary- 99.65
- Critical — this CVE ranks in the top fifth of all CVEs scored today (rank ≥ 80%).
KEV status
The KEV catalogue is a list maintained by the US authority CISA of vulnerabilities for which active exploitation in the wild has been confirmed. It answers not the question „how severe?“ but „is it already being attacked?“ — thereby sharply prioritising patching.

Response and mitigation
Why act now?
Prioritisation rationale
This is a low-complexity local privilege escalation. It needs only a low-privileged account, no user interaction, and yields full compromise of confidentiality, integrity and availability on the host. Attackers chain it after phishing or webshell access to reach SYSTEM, blind EDR and harvest credentials. Exploitation likelihood is very high and ransomware use is confirmed. The fix has been available since October 2021, so any remaining unpatched host signals a patch-management failure. Under NIS2, treat such hosts as critical findings, with terminal servers and admin workstations first.
Runbook step 1
Immediate response (0-24 h)
- Deploy the October 2021 Patch Tuesday cumulative update, or any later cumulative update, to all Windows clients. Fixed builds start at 10.0.10240.19086 (1507), 10.0.14393.4704 (1607), 10.0.17763.2237 (1809), 10.0.18363.1854 (1909) and 10.0.19041.1288 (2004/20H2/21H1). Update Windows 11 as well. Look up the exact KB per build in the MSRC advisory for CVE-2021-40449, and check Windows Server SKUs there too.
- Build a patch-gap inventory. Use MECM, Intune or WSUS reporting, or
Get-ComputerInfo | select OsBuildNumber, to list endpoints below those builds. Start with terminal servers, VDI and jump hosts that carry many interactive users. - Handle multi-user hosts first. On Citrix, RDS and AVD hosts, a local privilege escalation exposes every logged-on session and every cached credential. Drain unpatched hosts from the pool until they are updated.
- Assume escalation on hosts with initial-access alerts. For any endpoint already flagged for a phishing loader, webshell or RAT, assume SYSTEM was reached. Isolate the host, rotate local and cached domain credentials, and reset local admin passwords via LAPS.
- Account for the ransomware link. CISA flags known ransomware campaign use. Verify offline backups and have the recovery runbook ready.
Runbook step 2
Mitigation layers
- Endpoint hardening / application control: Enforce WDAC or AppLocker and block unsigned EXE/DLL execution from
%TEMP%,%APPDATA%,%PUBLIC%and Downloads. The exploit ships as a dropped binary and needs user-level code execution to run. - EDR protections: Enable ASR rules, especially "Block executable files unless they meet a prevalence, age, or trusted list criterion" and "Block credential stealing from LSASS". Turn on Tamper Protection and enforce Exploit Protection (CFG, ACG) on exposed processes.
- IAM / least privilege: Remove standing local admin rights and enforce admin tiering. Apply Credential Guard and the Protected Users group to privileged accounts. This limits the blast radius after a SYSTEM escalation: credential theft and lateral movement.
- Network segmentation: Block client-to-client SMB (445/tcp), RPC (135/tcp), WinRM (5985/5986/tcp) and RDP (3389/tcp) through host firewall policy or private VLANs, so an escalated workstation cannot be used as a pivot.
- Restrict printer functionality: The bug is a use-after-free in
NtGdiResetDC, triggered through a user-mode printer-driver callback. Restrict printer driver installation to administrators via Point-and-Print GPO. This is attack-surface reduction, not a substitute for the patch.
- Endpoint hardening / application control: Enforce WDAC or AppLocker and block unsigned EXE/DLL execution from
Runbook step 3
Detection rules
- Sysmon EID 1 / Security 4688 — integrity jump: Flag a child process running as SYSTEM whose parent runs at Medium integrity under a standard user and is not a service, installer or updater. KQL:
DeviceProcessEvents | where ProcessIntegrityLevel=="System" and InitiatingProcessIntegrityLevel=="Medium" and InitiatingProcessAccountName !in~ ("system","local service","network service") - EDR process ancestry: Flag a freshly written binary in a user-writable path that spawns
cmd.exe,powershell.exeorrundll32.exeas SYSTEM. Sigma shape:ParentImage|contains: '\AppData\' OR '\Temp\'ANDUser|contains: 'SYSTEM'. - Token manipulation and kernel instability: Watch EDR alerts for token theft or token swapping ("Suspicious token manipulation", "Kernel exploit"). Also watch for crashes in
win32kbase.sys/win32kfull.sys, visible as System log Event 1001 (BugCheck) and WER entries. Failed exploit attempts often cause blue screens. - Post-escalation activity: Security 4720/4732 (local account created or added to Administrators), 7045/4697 (new service), Sysmon EID 10 with
TargetImageending inlsass.exeandGrantedAccess0x1010/0x1FFFFF, and shadow-copy deletion (vssadmin delete shadows, wmic shadowcopy delete). - Correlation: Raise any initial-access alert followed by an integrity jump on the same host within 60 minutes to top severity.
- Sysmon EID 1 / Security 4688 — integrity jump: Flag a child process running as SYSTEM whose parent runs at Medium integrity under a standard user and is not a service, installer or updater. KQL:

Classification under NIS2 and CRA
Rule-based classification from exploitation status and severity. It does not replace a case-by-case assessment.
NIS2
NIS2 is EU Directive (EU) 2022/2555 to raise the level of cybersecurity across the Union. It substantially broadens obligations compared with the first NIS directive: more sectors, mandatory risk management, staged reporting duties and personal accountability of senior management.
Read in glossary- Actively exploited: treat the vulnerability as a priority in vulnerability handling under Art. 21(2)(e) NIS2. If it is exploited in your environment, check the reporting obligation under Art. 23 (early warning within 24 hours).
CRA
The CRA is EU Regulation (EU) 2024/2847 setting horizontal cybersecurity requirements for „products with digital elements“ — from industrial controllers and software to connected consumer devices. For the first time manufacturers must ensure security across the entire product lifecycle, handle vulnerabilities and provide security updates.
Read in glossary- Since 11.09.2026, manufacturers of affected products with digital elements must report actively exploited vulnerabilities under Art. 14 CRA, with an early warning within 24 hours via ENISA's single reporting platform.
ISO/IEC 27001
ISO/IEC 27001 is the internationally leading standard for an information security management system (ISMS). It describes a risk-based, process-oriented framework to systematically plan, implement, review and improve information security.

CWE-416Use After Free
Variant
The product reuses or references memory after it has been freed. At some point afterward, the memory may be allocated again and saved in another pointer, while the original pointer references a location somewhere within the new allocation. Any operations using the original pointer are no longer valid because the memory "belongs" to the code that operates on the new pointer.
